"Name" field title missing on options tab

Bug description

Title of Name field is not visible. I'm pretty sure this field used to have a title because I had filled it before, and it took me a moment to figure out how to add a name this time around.

Steps to reproduce

Opened the schedule card to configure a new or existing schedule and hit "options".
