The Recommendation 10 logic flow diagram condition logic for unexpected UDS results includes, for prescribable substances, an evaluation for active orders of that substance type in a patient’s med list. If an active order exists for the same substance type for which there is a positive result, the alert should be suppressed, and the response card should be empty. For example, if a patient has a positive test result for an amphetamine, and also has an active order for an amphetamine, the unexpected test result alert should be suppressed.

Per clinical site feedback, this approach has changed. When there is an unexpected result for a prescribable substance, the service should review the patient's medication history for medication orders in the preceding 30 days from the date of the test result. When there is an order for the substance that is identified as positive in the urine screen that occurs within a 30-day window prior to the test, the alert should be suppressed.
