The text here isn't clear on how to use the criteriaReference extension: https://build.fhir.org/ig/HL7/cqf-measures/measure-conformance.html#continuous-variable-measure

The Measure snippet 3-18 refers to an identifier section which isn't valid and isn't in the linked example resource.
