Bug #304 was caused by the CPU code mistakenly overwriting an instruction that was already present in the CPU pipeline.

This operation should never be possible, and it is preferrable to raise a runtime exception than allowing this overwrite to happen.

The Pipeline class should enforce this kind of preconditions.
